He declared that he resided at 83 Willis Street, Poplar E., London, England; that his wife was deceased; that he had a daughter, Violet Alice, who was born on February 6, 1913; and that his next of kin was his friend Mrs. Annie Morley, who resided at 17 St. Leonards Avenue, Poplar, London, England. He was described as 5 foot 6-1/2 inches in height; fresh complexion; brown eyes; brown hair. He had a tattoo on his right forearm.

John William Anderson received the British War Medal and Victory Medal.
