Attestation papers signed 23 May 1916 in Vanleek, Ontario. Writing on the sheet is very difficult to decipher. Thomas Townsend was born in London, England on 11 Jun 1898. He was a Farmer in Vanleek Hill, Ontario. His Next of Kin was his sister, Mrs Mable Whitfield of 41 Brettroad, Stonebridge Park, London, England. 5'2" in height with grey eyes and brown hair. His religion was listed as Presbyterian.

Commonwealth War Graves Registers states that Thomas died of wounds at No. 2 Canadian Field Ambulance. His Next of Kin was Mrs. B. M. Whitford, 5 Waverley Road, Paddington, Marrow Rd, W. 2. He was buried at Tigris Lane Cemetery, Wancourt, Nord Pas de Calais, France. Plot: II. B. 2.
