He declared that he was born in London, England; that he was married; that his mailing address was 219 West 42nd Street, New York, New York, USA; that his date of birth was February 25, 1890; that he was a citizen of Great Britain; that he was a travelling showman; that he was employed by Harry Witt, Victoria Attraction; and that he was Hebrew. He was described as having blue eyes; auburn hair; short stature; slender build. A notation on the draft form stated that a copy of this form was mailed in a stamped envelope c/o Harry Witt, Victoria Attraction, Watertown, New York.
