He declared that he resided at 202 Sherbourne Street, Toronto, Ontario, Canada; that he was born in Seaford, Sussex, England; that his next of kin was his wife, Mildred Alce, who resided at 202 Sherbourne Street, Toronto, Ontario, Canada; and that he was a Chauffeur. He was described as 5 foot 7-1/2 inches in height; dark complexion; blue eyes; dark hair. He was a member of the Church of England. He had 5 vaccination marks on his left arm.
