On August 7, 1915, George Chamberlain enlisted in the Canadian Armed Forces. He declared that he was born in Kingston-on-Thames, England; that his next of kin was Frank Hills, whose relationship to him was as a Guardian and who resided at 1073 Main Street E, Hamilton, Ontario, Canada; and that he was a market gardener by trade. He is described as 5 foot 3 inches in height; medium complexion; brown eyes and hair; and a Wesleyan by faith. He had four vaccination scars on his left arm.

On January 6, 1916, George Chamberlain re-enlisted in the Canadian Armed Forces. He declared that he resided at 1074 Main Street E., Hamilton, Ontario, Canada; that he was born in Kingston-on-Thames, England; that his next of kin was Frank Hills, whose relationship to him was as a Guardian; that he was a market gardener by trade; and that he had served for 8 weeks prior to his enlistment with the 91st C.H. Regiment Camp. He is described as 5 foot 3-1/2 inches in height; ruddy complexion; brown eyes; dark brown hair; and a Methodist by faith. He had five vaccination scars on his left arm.
