He declared that he resided at Shier Street, Bracebridge, Ontario, Canada; that his date of birth was April 6, 1896; that he was born in Wendover, England; that he was not married; that he was a member of the English Church; that he worked in the Saw Mills; that his next of kin was his mother, Mrs. Lydia Philbey, who resided at Shier Street, Bracebridge, Ontario, Canada.
